Roles and ResponsibilitiesAll skilled nursing sub-acute centers company-owned and operatedAs a Physical Therapist you willReview physicians referral and patients medical records to help determine diagnosis and physical therapy treatment required. Evaluate, care for and direct treatment for patients.Plan, prepare and perform individually designed treatment programs to maintain, improve andor restore physical functioning, alleviate pain andor prevent physical dysfunction in patients.Identify and document goals, anticipated progress and plans for re-evaluation.Evaluate effectiveness of treatment at various stages and adjusts treatment to achieve max benefit.Provide patients with a meaningful rehab background utilizing excellent clinical skills and customer service.QualificationsBachelors degree B.A. from a college or university accredited by the Commission on Accreditation of Physical Therapy Education CAPTE.Position mandates a min of 2 years job-related skills or equivalent combination of education and work background.
